Epidermal Nerve Fiber Density (ENFD) testing is a minimally invasive and highly specialized diagnostic procedure used to measure the density of small nerve fibers in the skin. These small nerve fibers extend from the papillary dermis into the skin’s epidermis, or surface epithelium, playing a vital role in sensory functions. ENFD testing is the recognized gold-standard diagnostic tool for assessing small fiber peripheral neuropathy, a condition that can manifest as chronic pain, burning sensations, or numbness.

By quantifying and evaluating the integrity of the terminal branches of peripheral nerves within the epidermis, ENFD testing provides objective evidence of small fiber damage. A diminished number of these small nerve fibers signals established nerve damage or disease, with lower counts correlating to more severe damage or advanced disease stages.

How is the Skin Punch Biopsy Performed?

The biopsy is minimally invasive and performed using a 3 mm punch tool. The patient’s skin is anaesthetized with 2% lidocaine with epinephrine, which alleviates the pain during the biopsy. The collection takes the provider approximately 90 seconds to perform, with the entire procedure taking about 10-15 minutes. Samples are placed in Zamboni fixative and shipped in pre-paid packaging. The biopsy sites are covered with a bandage and do not require any special treatment.
